EFFA has carefully written informative materials which we believe will aid in a better understanding of food allergies, and has developed healthcare forms to facilitate their management. 
Forms: Individual Healthcare Plan (IHP) Forms For School Use
        In our work, we have found a lack of IHP forms appropriate for students with 
        food allergies. We have responded to this need by designing a clear and concise 
        IHP form tailored specifically for this purpose.  The staff member responsible for 
        implementing each accommodation is clearly indicated on the form.  A planning 
        worksheet with a step-by-step format is also included to help facilitate this 
        process.  IHP packets include a Cover Sheet, and Accommodation Form, a Planning 
        Worksheet, an Emergency Treatment Plan, and a list of the Risks of allergen 
        exposure possible within each 'Focus Area' of the school environment.

        A blank form to be completed and used for the treatment of an allergic reaction.
